Coaches Disclosure Form
All coaches must submit to a background check in order to coach at Westfield Youth Soccer. In order to complete the disclosure form, coaches must log on to their gotsoccer accountby following this link: https://www.gotsport.com/asp/coaches/login.asp.
There is a link to "Background Checks" in the upper right once you login. Click on the "Background Checks" link, and on the Background Check
Information page, click on "Submit New Report."
There has been a lot of confusion over logins: your "coach" login username and password are not the same as the logins that were configured for player registration or family registration.
If you do not remember your username or password, follow the nstructions at the bottom of https://www.gotsport.com/asp/coaches/login.asp or contact the club and e will help.
